What Is Airway-Focused Orthodontics?

Airway orthodontics, sometimes called airway-focused orthodontics, is a treatment approach that considers how jaw structure, tongue posture, and dental alignment affect the upper airway. Traditional orthodontics focuses mostly on straightening teeth. Airway-focused care adds a clinical layer: can the patient breathe well through the nose, is there enough room for the tongue, and will the final bite support healthy sleep and airflow.

The connection is structural. A narrow palate, a recessed lower jaw, or crowded teeth reduce the space available for the airway. That can contribute to mouth breathing, snoring, fragmented sleep, and in more serious cases, obstructive sleep apnea (OSA). Obstructive sleep apnea is estimated to affect 22 to 26 million American adults, and up to 1 in 10 children show signs of sleep-disordered breathing (SDB). Orthodontics cannot solve every airway issue, but when the cause is structural, the right treatment plan can make a meaningful difference. Whether we ultimately use braces, Invisalign, or an expansion appliance, the plan starts with an airway assessment.

Peer-reviewed research links chronic mouth breathing in kids to narrower dental arches, a more retruded lower jaw, and reduced nasal airway volume. Catching these patterns early tends to mean shorter and simpler treatment later in life. For this reason, the American Association of Orthodontists (AAO) suggests an initial evaluation around age 7.

How We Evaluate the Airway

Cone beam computed tomography (CBCT) imaging is the key diagnostic tool. A traditional panoramic X-ray shows teeth and bone. A cone beam scan shows the airway in three dimensions, so we can measure whether the space behind the tongue is narrowed, whether the palate is high and constricted, and whether the lower jaw is set back in a way that reduces airway volume. Those measurements shape the treatment plan.

Airway issues caused by structural problems can often be improved with orthodontics. Issues caused by enlarged tonsils or adenoids, chronic allergies, or nasal obstruction may need a referral to an ear, nose, and throat (ENT) specialist, pediatrician, or sleep physician. Dr. Lee works with those specialists when that is the right call.

Treatment Options

No two patients are the same. The right plan depends on age, anatomy, and what the airway evaluation reveals. The treatments below cover the options we use most often.

Palatal expansion creates more space in the upper jaw and can help you breathe better. Kids often use rapid palatal expanders (RPE) to guide jaw growth, while adults can use newer options like mini-screw-assisted rapid palatal expanders (MARPE) and miniscrew expanders (MSE) to widen the jaw without surgery, which wasn’t available in the past.

Early interceptive treatment (Phase 1)

For kids aged 7 to 10, growth guidance appliances and limited braces can guide jaw development before problems become permanent. Our approach to Phase 1 care is planned around airway goals, not just tooth alignment.

Braces and clear aligners

Both can be planned with the airway in mind, so the finished bite supports good tongue posture and healthy breathing. Braces and Invisalign clear aligners are both viable, depending on the case. For many adults, a combination of expansion followed by aligners or braces produces the cleanest airway-aware result.

Mandibular advancement appliances (MAA)

For select patients, orthodontic and functional appliances can reposition the lower jaw forward, which opens the airway behind the tongue. This approach is most effective in growing patients and in carefully selected adult cases.

Myofunctional therapy

Targeted tongue and facial muscle exercises that train the tongue to rest against the palate, encourage nasal breathing, and help lock in orthodontic results. We coordinate with trained myofunctional therapists when this belongs in your plan.
